GENERAL ENROLLMENT INFORMATION

Thank you for your interest in enrollment at La Jolla High School.

2024-2025 ACADEMIC YEAR ENROLLMENT INFORMATION

Please contact Megan Cox at [email protected] to begin the enrollment process.

The following forms will need to be completed (see links below):

• K-12 Enrollment Form
• Universal Form
• Health Information Exchange Form
• Home Language Survey
• Request for Records

  In addition, please prepare a copy of the following documents:

• Proof of Residency/Address (utility bill, lease or mortgage)
• Immunization record(s)
• Birth Certificate or Passport
• Transcripts (please provide course descriptions from the School of attendance)
• Copy of IEP or 504 plan if applicable
